When it's not the piriformis

It is most likely the gluteus medius.

The gluteus medius is responsible for a large number of low back pain complaints.

It can cause pain while sitting, limping while walking, and decrease hip range of motion.

The therapeutic goal when faced with a gluteus medius issue is to release the antagonistic muscles which are the hip adductors and then work on strenghtening the gluteus medius
